Criteria

Applications will only be accepted from patient and professional organizations and associations located outside of the United States classified as a not-for-profit, or non-governmental organization (NGO) within their country and must abide by their local and regional laws. Participation is limited to where allowed by local law.

Consideration will NOT be given to individuals, medical facilities or research institutions. Nor will applications be accepted for pre-clinical, clinical or translational research programs.

Applications must demonstrate excellence in the following criteria categories consistent with the theme of Supporting Shared Decision Making in Hematology:

  • Inspired innovation
  • Measurable impact
  • Metrics for success

Applications should contain a solid rationale and background of the program/idea, the objectives, a description of methodology, and demonstrate the alignment with the defined theme.

Applications will be accepted for:

  • Existing program to be grown or expanded
  • New idea that requires initial funding

Additional Guidelines for submission:

  • Each organization may submit only one entry.
  • Applications must be submitted using the online application form found here.
  • Applications must be submitted in English.
  • Applications must be received by the deadline: Saturday, May 18, 2019
  • Applications must be complete to be eligible for consideration.
  • Applications must address the theme: “Supporting Shared Decision Making in Hematology.”
  • Applications must demonstrate excellence in the following criteria categories:
